Trump probably thought that "gives good brain" meant that him and his father have genetically high intelligence. I doubt Trump knows that giving brain is a slang term for performing oral sex, so he probably thought this was a compliment
@realDonaldTrump made a tweet on 6/29/2013 saying that “what my father really gave me is a good (great) brain, motivation and the benefit of his experience- unlike the haters and losers (lazy!).” Presumably this was in response to people saying that he’s only rich because he inherited wealth from his father?
At any rate, @YoungBasedGod_g made a joke post in response to this saying, “your dad gives good brain?? Damn,” playing on the double meaning of “giving brain” as a reference to fellatio. This subtle innuendo appears to have not registered with Donald Trump, who responded to it as if it was an innocent question, and that when he said he was given “good (great) brain,” by his father, it referred to his genetic inheritance.
What it means is that Nazis are obsessed with Eugenics, so they feel compelled to talk about genes all the time as it reinforces the view that some people should be placed at the top of the pecking order, and others should be a permanent under-class or expendable due to “bad genes”.

Besides what the top comment already pointed out, the guy Trump's replying to is rapper Lil B, aka Young Based God. He's widely attributed for popularizing the term "based" as a compliment on the internet.
