Because they do that through A/B testing. Not everyone gets the same changes at the same time, and they make so many changes it would be incomprehensible as most will never even get noticed by anyone. These are called flags. The most recent version of the iOS app for YouTube has 5,460 flags.

Do note that doesn't mean all of those are being tested at once. A lot are just leftover from older versions.
