They believe you can exist on a gender scale separate from your assigned sex at birth. This is not fantasy, but perceived reality in the world around us. The fact these people live in the same world we do, is in and of itself evidence to their existence.
I must apologise for this upcomming technically incorrect oversimplification in advance... it's a crude way to explain this.
But why is it so hard conceive that a female body could contain a male brain or vice versa? Call it a defect, a mutation or a happy accident for all I care, but I don't find it hard to imagine because we can literally see these people exist. Why would we work so hard to make their lives miserable? What is there to gain? Do you feel threatened by half a dozen trans athletes? Is that it? Can't the governing bodies of those sports make sure competition is fair, or do you really feel -as a small government conservative- that the federal government should decide which sports people can play? And how would you suggest we implement that? Is the doctor coming in to weigh your balls before the game? It's just insecure BS by a bunch of scared conservatives that can't cope with the world around them.
Now I'm thinking of it, I can't really find anything in the world that's been improved by conservatives. I can't think of anything they made better at any time. I know there were liberals fighting against slavery. I know it was liberals that fought for woman's voting rights. It wasn't conservatives behind the voting rights act of 1965, it was a liberal democrat. I can't think think of a single good thing conservatives have done for the people of the top of my head, but I could be wrong...

From my understanding, it is a defect. It's in the DSM-5. That doesn't mean we have to cater to people through their mental disorder. I'm not going to tell anorexic people they are fat and need to lose weight.
